Promotion Official Rules

1. Eligibility: The promotion is open to all patients of any United Dental Partners’ dental offices, aged 5 years or older at the time of entry. New and existing patients are eligible to participate. Employees and immediate family members of United Dental Partners are not eligible to enter.

2. Promotion Period: The promotion begins on October 4, 2023 at 7:00 AM Central and ends on October 31, 2023 at 7:59 PM Central.

3. Entry Method: To enter the promotion and have a chance to win an electric toothbrush, patients must: Schedule and attend a dental appointment during the specified promotional hours, which are 10am – 2pm Central on any available weekday during the promotion period. Entrants will automatically be entered into the promotion upon scheduling and attending their eligible dental appointment.

4. No Purchase Necessary: To enter the promotion without a scheduled appointment, please send email correspondence to marketingupdates@uniteddentalpartners.com. Please include your name and mailing address. Emailed entries must be received by October 31, 2023 at 7:59pm Central. 

5. Prize: Twenty-five (25) prizes will be awarded at the end of the promotion period. The prize consists of Oral-B Smart 1500 electric toothbrush ($44 value). The prize is non-transferable and may not be exchanged for cash or any other item. United Dental Partners reserves the right to substitute the prize with an item of equal or greater value if necessary.

6. Winner Selection: The winners will be selected randomly from all eligible entries. The drawing will take place on November 15, 2023. The winners will be notified by email within 5 business days of the drawing. If a winner cannot be contacted within 5 business days or is ineligible, an alternate winner will be selected.

7. General Conditions: United Dental Partners reserves the right to disqualify any entry that violates these rules or is considered fraudulent. By participating in the promotion, entrants agree to abide by these official rules and the decisions of United Dental Partners, which are final. United Dental Partners is not responsible for lost, late, incomplete, or misdirected entries. United Dental Partners is not responsible for any technical issues that may affect entry into the promotion. United Dental Partners is not responsible for the condition, functionality, or durability of any prizes awarded in this promotion. Prizes are provided “as is,” without any warranty or guarantee of any kind, either expressed or implied. United Dental Partners shall not be held liable for any defects, malfunctions, or damages that may occur with the prize, including but not limited to, electrical or mechanical failures, damage during shipping, or any other unforeseen issues. The winners acknowledge that they assume all risks associated with the use and possession of the prize.

8. Privacy Policy: Information collected from entrants will be subject to United Dental Partners’ privacy policy.

9. Governing Law: This promotion is governed by the laws of the city of Chicago in Cook County, IL.
